How to request your health records, reach our licensing board, and file a complaint, step by step.
Consumer access to health records and consumer complaints
Pursuant to Texas House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, please be advised of the following. This notice is posted here and at our office.
A. How to request access to health care records from New Leaf Chiropractic

How to submit a request:

What to include with your request:
- Your full name and date of birth
- A phone number where we can reach you
- The dates of service or records you are requesting
- The format you prefer (paper or electronic)
- Where the records should be sent, if not picked up in person
We will verify your identity, or your legal authority if you are requesting records as a personal representative, before releasing records.
Timeframe for our response: electronic records are provided within 15 business days of our receiving your written request, the timeline Texas law sets, and usually sooner. Other formats are provided within 30 days. A reasonable, cost-based fee may apply as allowed by law.
